Terms of Service

These Terms govern your use of the Fountain of Youth website, intake forms, and services provided by Lee Health Care Services LLC d/b/a Fountain of Youth ("we," "us"). By using our site or submitting an intake, you agree to these Terms.

1. What we are — and aren't

We are a telehealth medical practice. Our licensed providers review health intakes and, when medically appropriate, prescribe treatment. We are not an emergency service. If you are experiencing a medical emergency, call 911. Nothing on our website is medical advice until a licensed provider has reviewed your individual information.

2. Eligibility

You must be 18 or older, reside in the United States, and provide accurate, complete information in your intake. Providing false health information endangers your safety and is grounds for immediate termination of care.

3. Prescriptions are never guaranteed

Completing an intake, creating an account, or paying any fee does not guarantee a prescription. Prescribing is an individualized medical decision made by a licensed provider. If a treatment is not appropriate for you, we will tell you and you will not be charged for medication.

4. Telehealth consent

By submitting an intake you consent to receive care via telehealth, including asynchronous review of your intake by a licensed provider, and — where your state requires or your provider determines — a phone or video visit. Telehealth has limitations compared with in-person care; you may be referred for in-person evaluation when appropriate. You may withdraw consent at any time by contacting us, which may end our ability to treat you.

5. Compounded medications

Medications we prescribe through this program are compounded by Red Rock Pharmacy, a state-licensed 503A compounding pharmacy, and shipped to you directly by the pharmacy or dispensed through our clinic. Compounded medications are not FDA-approved. They are prepared by a state-licensed compounding pharmacy. We do not manufacture or compound any medication. Individual results vary; no specific outcome is promised.

6. Subscriptions and billing

Wellness plans are billed monthly by subscription through Stripe, beginning only after a provider has approved your prescription and you choose to start. Plans renew automatically each month until you cancel. Bundle and add-on pricing requires the qualifying active plan; if it lapses, standard pricing applies. 7. Your responsibilities

Keep your health information current (tell us about new diagnoses, medications, pregnancy or breastfeeding status, or side effects), use medications only as directed, store them properly, and never share or resell prescription medication — it's dangerous and illegal.

8. Intellectual property and acceptable use

Our content, branding, and materials belong to us. Don't scrape, copy for commercial use, attempt to access other patients' information, or misuse the site.

9. Disclaimers and limitation of liability

To the fullest extent permitted by law, the website and non-clinical services are provided "as is," and our total liability arising from use of the website or these Terms is limited to the amounts you paid us in the twelve months preceding the claim. Nothing in these Terms limits liability that cannot be limited by law or alters the professional obligations our providers owe you under applicable medical-practice laws.

10. Governing law and disputes

These Terms are governed by the laws of the State of Texas. Disputes will be resolved in the state or federal courts located in Collin County, Texas, except where applicable law gives you rights to another forum.

11. Changes

We may update these Terms; continued use after changes are posted constitutes acceptance. Material changes will be flagged on this page.
