Privacy Policy

Fountain of Youth is a DBA of Lee Health Care Services LLC ("we," "us"). This policy explains what information we collect when you visit our pages, complete a health intake, or become a patient — and what we do with it.

What we collect

• Contact information: name, email, phone, mailing address, date of birth, state of residence.
• Health information: your health intake responses, medical history, medications, allergies, and information generated during your care (prescriptions, provider notes, messages with our clinical team).
• Payment information: payments are processed by Stripe. We do not store your full card number on our systems.
• Technical information: on our general marketing pages, standard analytics data (browser, device, pages visited). We do not place advertising pixels or third-party trackers on our health intake or patient pages.

How we use it

• To provide medical care: provider review of your intake, prescribing decisions, and ongoing clinical support.
• To fill prescriptions: if a provider prescribes for you, we share your prescription and shipping information with the dispensing pharmacy (Red Rock Pharmacy, a state-licensed compounding pharmacy).
• To run your account: billing through Stripe, appointment or refill reminders, responses to your messages.
• To communicate with you by email and — only if you opted in — by SMS. Reply STOP to any text to opt out.

What we do NOT do

• We do not sell your personal or health information. Ever.
• We do not share your health information with advertisers or social media platforms.
• We do not use your health intake answers for marketing.

Health information and HIPAA

As a healthcare provider, we protect your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law. We share it only: with the pharmacy filling your prescription; with members of your care team; with service providers under appropriate agreements who help us operate (e.g., our practice-management platform); when required by law; or with your written authorization. You may request a copy of your records, request corrections, or ask us to restrict certain disclosures by contacting us below.

Retention and security

We retain medical records as required by state medical-records laws. We use access controls, encryption in transit, and vendor agreements designed to protect your information. No system is perfectly secure; if a breach affecting your information occurs, we will notify you as required by law.

Your choices

• Unsubscribe from marketing emails via the link in any email.
• Reply STOP to opt out of texts at any time (this does not affect your care).
• Contact us to access, correct, or request deletion of your information (medical-record retention laws may limit deletion).

Children

Our services are for adults 18 and older. We do not knowingly collect information from anyone under 18.

Changes

We may update this policy; the effective date above will change and material changes will be noted on this page.
